No, you do not have to speak Spanish at all if you do not want to, as English is widely spoken in touristy places, unless you are planning to spend a substantial amount of time in the countryside, or visit the rural areas extensively. Costa Rican people are generally friendly and outgoing, and will go out of their way to help the tourists in need, even if the later do not speak Spanish. Also, in view of the current social and economic development in the country, the Costa Rican education ministry has in recent times introduced various incentives for working Ticos to learn English as a second language, which is already picking up a lot of momentum in the commercial sectors.
Nonetheless, learning to speak a few Spanish words and phrases will greatly enhance your vacation experience, it will open more avenues for you to experience the local culture and encourage you to interact with the local people. Even with a few basic words, you can bring a smile to a Tico’s face, such as "Gracias" (thank you), "Por favor" (please) and "Mucho Gusto" (It's a pleasure), or greeting them with "Buenos dias" (good morning) or “Buenas tarde” (good afternoon). If you can only manage to learn just one phrase, just remember to say “Pure Vida” when you mean a person, an object or a situation is great, cool, superb or fantastic!
